It is the policy of Michigan Heart to comply with the requirements of the HIPAA Privacy Rules. This includes Michigan Heart's compliance relating to: the use or disclosure of patient health information; notification to patients of their rights under the HIPAA Privacy Rules; recognizing and respecting patients' rights to access, amend or request and accounting of their health information pursuant to the HIPAA Privacy Rules; proper efforts relating to the physical, administrative and technical security of patient health information and records; and ensuring that others with whom Michigan Heart contracts agrees to act in a manner to ensure Michigan Hearts compliance with the HIPAA Privacy Rules.

Michigan Heart takes compliance with patient privacy, confidentiality and other requirements of HIPAA Privacy Rules seriously. Accordingly, incorporated by reference into this policy - and Michigan Heart's Compliance Program, as a whole - are the attached policies, procedures, protocols, notices, forms, agreements and other documents Michigan Heart has developed and instituted to become and stay compliant with the HIPAA Privacy Rules.
